
If you've Googled “is BHRT safe” at 11pm and closed forty tabs more confused than when you started — you're not alone. And you're not wrong to be careful.
The hormone safety conversation has been shaped by:
• One 23-year-old study that didn't even use bioidentical hormones
• A flood of no-monitoring telehealth scripts in the last few years
• A lot of well-meaning, oversimplified advice from friends
Below are the 6 questions patients ask me most — answered the way I'd actually answer them in a visit: real data, real nuance, and the exact labs we use to keep you safe.
“I heard hormones cause breast cancer.”
This fear traces back almost entirely to one study: the Women's Health Initiative (WHI), published in 2002.
Here's what most people never hear about it: it used oral conjugated equine estrogens (Premarin) and synthetic medroxyprogesterone acetate (Provera) — not bioidentical estradiol and micronized progesterone. Different molecules, different results.
And there's a second correction that matters even more: when researchers later stratified the data by age at initiation, women who started hormone therapy near the onset of menopause showed a cardiovascular benefit — not just an absence of risk. The scary headline came from women who started years later, on the wrong molecules, at the wrong dose.
What I do differently: My protocols are built on Dr. Neal Rouzier's clinical teaching — dosing to relieve symptoms and restore physiologic hormone levels, not just clearing a lab's lower threshold. Before any prescription, I run baseline labs (including hormones and lipids) and a personal/family cancer history review. Then I re-check on a schedule, because under-treating gets just as much attention here as over-treating.
“Isn't BHRT unregulated and not FDA-approved?”
Partially true — and worth being precise about, because the two categories work very differently.
FDA-approved: Some bioidentical hormones — estradiol patches and oral micronized progesterone (Prometrium), for example — are manufactured products with batch-level quality control.
Compounded: Others are custom-mixed by a licensed pharmacy to your specific dose or delivery method. These fall under USP compounding standards rather than FDA drug approval — a real distinction, which is exactly why the pharmacy behind your prescription matters as much as the hormone itself.
What I do differently: I use what I know is most effective, depending on your labs and goals, and I source compounded formulations only from PCAB-accredited pharmacies. I'll always tell you plainly which category your prescription falls into and why.
“A friend tried BHRT and had a bad experience.”
Most of the bad outcomes I hear about secondhand trace back to one root cause: no monitoring.
The recent surge of direct-to-consumer telehealth hormone and GLP-1 companies has made starting a prescription easy — a questionnaire, a quick video call, a script. The follow-up lab work and dose adjustment that actually prevents problems is frequently where those models fall short. It's also why federal regulators have issued warning letters to several telehealth prescribers over the past year for inadequate clinical oversight.
What I do differently: Every patient gets a structured re-check schedule — typically 6-8 weeks after starting or adjusting a dose, then at defined intervals after that — tracking estradiol, free and total testosterone, SHBG, and relevant metabolic markers, not just a symptom checklist.
“Why would my dose need to be higher than what my last doctor prescribed?”
This comes up constantly with patients who've tried hormone therapy elsewhere and felt little to no improvement. Most conventional prescribing dosed conservatively to the bottom of a lab range and stopped there — which often means staying just barely out of deficiency, not actually reaching the level where symptoms resolve.
My approach, trained under Dr. Neal Rouzier, aims for symptom relief and physiologic optimal range. For many patients, that means a meaningfully different dose than what they were given before.
What I do differently: I don't anchor your dose to the lowest number that technically clears a deficiency cutoff. I anchor it to your symptoms, your labs, and your response.
“Won't this raise my risk of blood clots or stroke?”
Here's the distinction I learned from Dr. Neal Rouzier that most patients are never given: the clot and stroke risk people worry about almost entirely traces back to one specific kind of estrogen — not estrogen as a category, and not the kind I actually prescribe.
The Women's Health Initiative study that started this whole fear used conjugated equine estrogen (Premarin) — a synthetic, non-human molecule — paired with synthetic medroxyprogesterone acetate (Provera). Bioidentical estradiol is structurally identical to the estrogen your own body produces, and it behaves differently in your bloodstream. Rouzier points to the French E3N and ESTHER cohort studies as the clearest evidence here: women using bioidentical estradiol showed a meaningfully different clot risk profile than women on the synthetic version that scared everyone in 2002.
Testosterone gets lumped into this fear too, but it's an entirely different molecule with a different risk profile altogether. The conversation about testosterone and cardiovascular risk is worth having — it just isn't this conversation.
So the honest answer is: it depends entirely on which estrogen you're taking and what your personal risk factors are. “Hormones” as a category don't raise your clot risk. The synthetic, non-bioidentical version studied in 2002 does.
What I do differently: I prescribe bioidentical estradiol, never synthetic conjugated estrogens — that choice alone removes the mechanism behind the WHI-era fear. I still don't stop there: I build your plan from your CAC score, Lp(a), ApoB, and personal clotting/cardiovascular history, the same cardiometabolic workup I run for every BHRT patient regardless of why they came to me.
“How would I even know if it's working, or if something's wrong?”
This is the right question — and it's the one most programs answer the worst. Feeling better is a real signal, but it isn't sufficient on its own.
I pair how you feel with objective markers — and unlike a standard primary care lab review, I'm not just checking whether you fall “in range.” Conventional reference ranges describe a population that includes plenty of fat, chronically ill, symptomatic, and deficient people. The goal in Rouzier-based dosing is the optimal physiologic range associated with symptom relief and protection — often mid-to-upper range, not the floor.
Markers I track, on a defined schedule — not just “as needed”:
✓ Estradiol, free/total testosterone, progesterone, SHBG
✓ ApoB, Lp(a), and a full lipid panel — not just total cholesterol
✓ hs-CRP for vascular inflammation
✓ CAC scoring when cardiovascular risk factors warrant it
✓ Symptom and HRV trends, often cross-referenced against your own Oura or wearable data
The Bottom Line
BHRT isn't inherently risky or inherently safe — it's only as safe and as effective as the workup, the dosing philosophy, and the monitoring behind it.
