My functional medicine concierge membership requires a 3-month minimum commitment.
We are a consultative practice and ask that you maintain a primary care physician.
Please allow up to 2 business days for a response. My office is not equipped to handle 24/7 calls, does not replace urgent or emergency care, and does not offer unlimited messaging services.
We provide superbills for you to submit to insurance for reimbursement if you choose.
We practice based on mutual respect and collaboration.

What is concierge adjunctive integrative medicine?
This approach combines the personalized attention and enhanced access of a concierge medical practice with the holistic and comprehensive principles of integrative medicine. "Adjunctive" means it works alongside your conventional medical care to offer complementary therapies and a broader perspective on your health.
Is this membership a replacement for my primary care physician?
No, this membership works in conjunction with your primary care doctor and specialists, offering a complementary and holistic approach. It focuses on wellness, prevention, and integrating various therapies.
What types of conditions or health goals can this membership help with?
Functional medicine can address a wide range of conditions and wellness goals, including chronic pain, fatigue, digestive issues, stress management, autoimmune disorders, and optimizing overall health and well-being.
How do you bill members?
When signing up for membership, you’ll be charged the first monthly payment on the date you sign up, and every month following that.
How do I contact my practitioner outside of scheduled appointments?
You can contact us via dedicated phone/text line, or patient portal.
Can my Functional Medicine Physician be my Primary Care Provider (PCP)?
No. Our physicians specialize in Integrative Medicine and work adjunctively with your PCP for your care.
Do you bill health insurance?
No, the out-of-pocket fee for your office visit is $350 for your initial visit and $115-$230 for follow up visits. This fee is on top of your membership fee.
How do I cancel my membership?
All subscribers may cancel their membership at any time during their 12-month agreement. The cancellation will take effect 30 days from the date of written notification. To request cancellation, email hello@brillianthealthinstitute.com.
If you make monthly membership payments, the monthly installments will stop 30 days after notification, and any payment scheduled during that 30-day period will process as normal.
Your membership will remain active during this time and through the end of the period covered by the final payment.
If the subscriber withdraws their membership and later decides to rejoin within 12 months of cancellation, there will be a $250 rejoin fee in addition to the regular membership fee.
